A newly established Lebanese cohort of 1,871 adults aged 50+ (mean age 65.6, 70.1% women) is now tracking whether participation in structured late-life learning programs protects against cognitive decline and dementia. The 3LC study recruited both enrollees in the American University of Beirut's University for Seniors program and age-, sex-, and education-matched community controls, with blood biomarkers collected from 82.3% of participants. Assessments capture war exposure, socioeconomic adversity, and psychosocial factors alongside standard cognitive and health measures, with two-year follow-up intervals planned.

This cohort addresses a genuine gap: nearly all major longitudinal aging studies originate from high-income, Western settings, leaving the Middle East and North Africa severely underrepresented in dementia research. Lebanon's layered stressors — war, economic collapse, displacement — create a naturalistic experiment for examining how adversity across the lifecourse shapes late-brain resilience, a question no Western cohort can adequately answer. The late-life learning exposure contrast is particularly compelling, as structured cognitive engagement has shown protective signals in European and American data but lacks causal confirmation anywhere.

That said, this is a study design paper, not an outcomes paper — no cognitive aging findings are reported yet. The predominantly female sample and urban Beirut recruitment limit generalizability within Lebanon itself. As a preprint not yet peer-reviewed, the methodology described here may be refined before publication. Still, the infrastructure built — biobank, matched design, conflict-exposure data — positions 3LC as an incrementally important but potentially field-expanding resource for global dementia prevention research.
