A narrative review from Semmelweis University synthesizes cross-disciplinary evidence positioning the gut microbiome as a central biological interface through which the Planetary Health Diet (PHD) — a largely plant-forward, environmentally sustainable eating framework — exerts its anti-aging effects. PHD-consistent dietary patterns correlate with greater microbial diversity, enhanced production of bioactive metabolites (including short-chain fatty acids and bile acid derivatives), and improved intestinal barrier integrity, immune tone, and metabolic function. These microbiome adaptations are proposed to modulate established hallmarks of aging, reducing risks of cardiometabolic disease, frailty, cognitive decline, and premature mortality.

The PHD, originally formalized by the EAT-Lancet Commission in 2019, was designed primarily around environmental sustainability targets — this review's value lies in mechanistically grounding its health benefits in microbiome science rather than epidemiology alone. The framing matters: if the gut microbiome is genuinely upstream of multiple aging hallmarks, dietary interventions targeting microbial ecology become geroscience tools, not merely public health recommendations.

That said, critical limitations demand honesty. This is a narrative — not systematic — review, meaning selection bias in source literature is plausible. Mechanistic pathways are largely inferred from associative cohort data and short-duration RCTs; causal directionality from diet to microbiome to aging outcomes remains incompletely established in humans. Microbial diversity metrics also vary substantially across populations. For health-conscious adults, the practical signal is confirmatory: diverse, plant-rich dietary patterns with minimal ultra-processed foods remain among the highest-confidence longevity levers available.
