Despite enormous international investment in road safety over three decades, the gap between ambition and reality on global roads remains troubling — particularly for the billions of people in low- and middle-income countries who bear a disproportionate share of the harm. Understanding exactly how that burden has shifted, and where intervention gaps persist, is now more urgent than ever as the world's 2030 road safety targets approach.

This comprehensive GBD 2023 analysis estimated incidence, mortality, disability-adjusted life-years (DALYs), years of life lost (YLLs), and years lived with disability (YLDs) for road injuries across 204 countries and territories spanning 1990 to 2023. The study disaggregated findings across four road-user categories and 47 nature-of-injury classifications, drawing on clinical records, vital registration data, and police reports harmonized through meta-analytic techniques to correct for systematic reporting biases. Mortality was modeled using the Cause of Death Ensemble model, while incidence relied on Disease Modelling-Meta-Regression v2.1, both incorporating location-specific covariates to improve interpolation across data-sparse settings.
