For older cardiac patients, knowing their frailty status at the time of coronary angiography may be just as clinically relevant as knowing their diagnosis — but the implications differ sharply depending on whether the coronary disease is acute or chronic. This distinction has important consequences for how cardiologists weigh procedural risk, treatment intensity, and prognosis conversations with patients over 70.

This prospective cohort study enrolled 678 patients aged 70 and older undergoing coronary angiography, divided between those presenting with acute coronary syndrome (ACS, n=455) and chronic coronary syndrome (CCS, n=223), with 12-month follow-up. Frailty was measured using the Clinical Frailty Scale (CFS), with scores of 4 or above classified as frail or vulnerable. Strikingly, frailty prevalence was actually higher among CCS patients (43%) than ACS patients (32%), yet it was only in the ACS group that frailty independently predicted elevated all-cause mortality over the follow-up year. Frail CCS patients were also younger on average and received percutaneous coronary intervention less frequently than their ACS counterparts.

This finding challenges a simplistic view of frailty as a uniform prognostic tool across cardiac presentations. In the broader research landscape, frailty has been increasingly validated as a mortality predictor in heart failure, post-TAVR populations, and general cardiovascular cohorts — but syndrome-specific calibration has been underexplored. The divergence here may reflect physiological differences: ACS patients face acute hemodynamic stress that exposes the limited physiological reserve characteristic of frailty, while CCS patients have adapted to more chronic, stable ischemic burden. The lower rate of revascularization in frail CCS patients also raises questions about whether clinical decision-making already implicitly accounts for frailty without formally measuring it. Limitations include the observational design, single-center characteristics, and a CFS tool that, while practical, relies on clinical judgment rather than objective performance metrics. Overall, this is an incrementally important contribution suggesting that frailty screening protocols in older cardiac patients may need syndrome-specific interpretation frameworks rather than a one-size-fits-all threshold.
