A 39-year-old man with catastrophically elevated triglycerides peaking at 2,329 mg/dL — nearly 13 times the upper reference limit — experienced treatment failure across statins, fibrates, and omega-3 fatty acids over nine years and six pancreatitis episodes. After cautious initiation of semaglutide, triglycerides fell to 213 mg/dL, a 91% reduction, alongside insulin cessation, hypoglycemia resolution, weight loss, and measurable improvement in liver fibrosis markers — all without pancreatitis recurrence.

This case carries outsized clinical significance precisely because it directly challenges a longstanding contraindication. GLP-1 receptor agonists have carried pancreatitis warnings since early post-marketing surveillance raised signals, leading most guidelines and clinicians to treat prior pancreatitis as a hard stop. Yet the underlying biology here is instructive: severe hypertriglyceridemia is itself a leading cause of pancreatitis, and semaglutide's lipid-lowering mechanism — reducing hepatic VLDL secretion and enhancing lipoprotein lipase activity — targets the root driver rather than masking it. The risk calculus, in a patient whose disease was progressing on maximal conventional therapy, arguably favors intervention.

Critically, this is a single case report, not a controlled trial — causality cannot be firmly established, and idiosyncratic responses limit generalizability. Nevertheless, it adds to a thin but growing literature suggesting GLP-1 agonists may be net protective in select hypertriglyceridemia-driven pancreatitis patients. For clinicians managing refractory cases under multidisciplinary supervision, this finding is paradigm-nudging if not yet paradigm-shifting.
