For millions of adults living with joint pain, corticosteroid injections have long been a frontline intervention — but the clinical assumptions underlying how these injections are administered may be significantly flawed. A newly published multi-society consensus guideline is now challenging the dosing norms that pain practitioners have followed for decades, with direct implications for patient safety and long-term joint health.
The guideline, developed through a modified Delphi consensus process by four major pain and anesthesia societies — including the American Society of Regional Anesthesia and Pain Medicine — synthesized evidence on intra-articular and peri-articular corticosteroid (IACS) injections across musculoskeletal regions. A key finding: the doses currently used in clinical practice routinely exceed levels shown in the literature to be effective. The guideline also documents adverse events occurring even from single injections, a risk profile that has historically been downplayed. Evidence for short-term pain relief is broadly supported, but long-term efficacy data remain mixed, and the benefit-to-risk calculus shifts meaningfully over repeated administrations.
This guidance arrives at a critical moment in musculoskeletal medicine. A growing body of imaging research — including studies showing accelerated cartilage loss and subchondral bone changes following repeated IACS injections — has been accumulating over the past decade without translating into standardized clinical practice changes. The new guideline's grading methodology, adapted from the U.S. Preventive Services Task Force framework, adds rigor often absent from specialty-society recommendations. Still, important limitations apply: much of the underlying evidence is observational, heterogeneous in methodology, and varies considerably by anatomical site. The guideline does not resolve the fundamental tension between patient demand for rapid relief and the structural risks of repeated steroid exposure. Clinicians and patients alike would benefit from greater transparency about these trade-offs — particularly for commonly injected sites like the knee and shoulder, where the cumulative harm data are most concerning. This represents an incremental but meaningful regulatory signal within pain medicine.
