For the millions of older adults who take statins not because they've had a heart attack but simply as a preventive measure, a foundational question has gone unanswered for decades: does continuing this medication past age 75 actually extend life? A rigorous new phase 3 trial published in The Lancet Healthy Longevity now offers the most direct evidence yet — and the answer challenges the default clinical assumption that statin therapy should continue indefinitely.

The SAGA/SITE trial enrolled adults aged 75 and older who had been on statins for primary prevention of atherosclerotic cardiovascular disease (ASCVD) for at least one year, with no prior history of the disease. Conducted across 297 primary care offices in a pragmatic, real-world design, participants were randomized in a 5:4 ratio to either continue or discontinue their statin. The three-year follow-up tracked all-cause mortality as the primary endpoint in a non-inferiority framework — meaning the trial was designed to determine whether stopping statins was no worse than continuing them for overall survival. Individuals with dementia, a life expectancy under three months, or familial hypercholesterolaemia were excluded, meaningfully sharpening the population's relevance.
