Population growth is quietly erasing decades of public health victories against tobacco exposure. Even as age-standardised rates of second-hand smoke (SHS) exposure have fallen meaningfully since 1990, the raw number of people inhaling others' tobacco smoke has not declined — a stark reminder that per-capita improvements can mask growing absolute harm in an expanding global population.

The GBD 2023 systematic analysis — covering 204 countries and territories across 33 years — estimated that 2.71 billion people were exposed to SHS in 2023, including 767 million children under 15. Age-standardised prevalence dropped 22.2% globally since 1990, yet this was insufficient to shrink the total exposed population. The burden is geographically concentrated: southeast Asia, east Asia, and Oceania reported the highest age-standardised prevalence at 48.4%. A striking sex disparity emerged, with women in certain countries experiencing SHS prevalence nearly 1.8 times that of men — likely reflecting indoor household dynamics where women spend more time in smoke-laden domestic spaces. The analysis also expanded its disease attribution framework to now include asthma alongside eight previously tracked outcomes, using the GBD's Burden of Proof methodology to generate more conservative, credible relative risk estimates.

This is a paradigm-clarifying finding for the field. Prior SHS analyses were fragmented by geography or outcome; this is the first dedicated GBD publication to consolidate SHS as a standalone risk factor at global scale. The inclusion of asthma meaningfully widens the attributable DALY calculation, suggesting the true disease burden has been systematically underestimated. From a longevity and healthspan perspective, SHS exposure in childhood carries compounding risk — impairing lung development and elevating lifetime cardiovascular and respiratory disease trajectories. The female-specific exposure pattern points to a structural inequity that smoke-free legislation alone may not resolve without addressing domestic norms. Key limitations include reliance on survey-derived exposure estimates, which may undercount informal or outdoor exposure in low-income settings. Still, the scope and methodological rigor here make this a landmark reference point for researchers and policymakers alike.
