A pragmatic cluster-randomized controlled trial — the SCOPE study — is enrolling 104 nursing home residents aged ≥75 with confirmed sarcopenia to compare three exercise protocols: modified Baduanjin (traditional Chinese qigong), resistance training, and a combined approach, against a control group. The 48-week caregiver-led intervention uses video-guided sessions with phased reduction in specialist supervision. The primary endpoint is a composite sarcopenia Z-score integrating handgrip strength, Short Physical Performance Battery scores, and appendicular skeletal muscle mass index, with follow-up extending to 2 years post-intervention.
The significance here lies in the population being studied, not just the intervention. Virtually all high-quality exercise-sarcopenia trials have enrolled community-dwelling older adults capable of self-selecting into research — a cohort fundamentally different from frail, institutionalized oldest-old individuals. Translating those findings directly to nursing homes has always been methodologically suspect. This trial closes that gap by embedding the intervention within the actual care workflow, using caregivers rather than specialists as the delivery mechanism — a pragmatic design choice that, if effective, could scale across facilities without prohibitive cost.
The inclusion of saliva biomarkers and neuromuscular activation outcomes alongside functional measures adds mechanistic depth rarely seen in nursing home exercise trials. Limitations are notable: single-facility recruitment in Hangzhou constrains generalizability, and 104 participants across four arms yields modest statistical power. This is still a protocol paper — no outcome data exist yet. Nonetheless, the design is thoughtful and addresses a genuine gap. Incremental but strategically important.
