When a sports governing body rewrites the rules of contact for female athletes, the injury consequences deserve rigorous scrutiny — especially for head trauma. The Swedish Women's Hockey League's 2022 decision to permit body checking created a natural experiment unique in the global history of women's ice hockey, and the first two years of data offer a cautionary, if preliminary, signal about concussion risk.

Across the 2022–23 and 2023–24 SDHL seasons, 46 game-related injuries were recorded among elite female players, yielding an overall game injury rate of 17.2 per 1,000 player-hours. Concussions emerged as the single most frequent injury type, comprising 22% of all events (n=10) at a rate of 3.7 per 1,000 player-hours. Body checking was identified as the leading individual injury mechanism, accounting for 20% of incidents. A striking finding was the temporal clustering of injuries: 46% occurred in the third period, with nine of those concentrated in the final six minutes — a pattern potentially reflecting fatigue-driven biomechanical vulnerability. More concerning from a player-safety standpoint, 50% of concussed athletes continued playing despite sustaining the injury, suggesting meaningful gaps in sideline recognition or reporting culture.

Placing these numbers in context requires restraint. The absence of a comparator cohort from the pre-checking era or from non-checking women's leagues makes causal attribution impossible; we cannot confirm that body checking, rather than confounding factors like increased competitive intensity, drives the concussion rate. The sample of 46 injuries is too small for robust inference, and the authors acknowledge incomplete reporting. Concussion rates in men's elite hockey typically range from 1.5 to 6 per 1,000 player-hours depending on league and methodology, meaning the SDHL figure sits within that male-sport range — itself an observation warranting careful follow-up as the dataset matures. This study is best read as a baseline-establishing descriptive exercise rather than definitive evidence. Its value lies in initiating systematic injury surveillance in a genuinely novel policy context, providing the foundation against which future seasons can be meaningfully measured.
