The addition reflects emerging clinical validation data showing that cell-free DNA shed by colorectal tumors into the bloodstream can be detected with sufficient sensitivity to warrant inclusion in a tiered screening framework. The guideline continues to position colonoscopy and high-sensitivity stool-based tests as preferred options, particularly for average-risk adults, while positioning the blood test as an alternative for individuals unlikely to complete other forms of screening.
From a population health standpoint, this is a pragmatic rather than paradigm-shifting move. The ctDNA liquid biopsy concept has been building momentum across oncology for nearly a decade, and its extension into colorectal screening reflects accumulated evidence rather than a sudden breakthrough. Key limitations deserve attention: blood-based ctDNA tests for colorectal cancer currently show lower sensitivity for early-stage lesions and precancerous polyps compared with colonoscopy, meaning that a negative result carries different clinical weight depending on the platform. Positive results still require confirmatory colonoscopy. The practical question is whether expanding optionality genuinely increases screening uptake among non-adherent populations, or whether it primarily redirects already-engaged patients toward a more convenient but less sensitive alternative.
