Analyzing Global Burden of Disease 2023 estimates across 204 countries, this study tracked each nation's episode-fatality ratio (EFR) for childhood lower respiratory infections (LRI) against a regional best-performance frontier over 33 years. The median country gap barely budged — 1.86 in 1990, 1.80 in 2019, back to 1.86 in 2023 — and 46.6% of countries remained more than twice their regional frontier by 2023. China was a standout improver (gap: 2.43→0.50), while the United States doubled its gap (×2.0) and Uzbekistan reached a staggering 28.6× its frontier. Sub-Saharan Africa's frontier fell from 4.2× to 9.5× behind the high-income frontier, meaning within-region benchmarking dangerously masks absolute deprivation.
These findings matter because LRI — primarily pneumonia — kills more children than any other infectious disease globally. The complete absence of convergence over three decades indicts systemic failures in health-system investment, oxygen access, antibiotic stewardship, and vaccine rollout rather than any single clinical gap. The divergence of the United States is particularly notable given its per-capita health expenditure. Nigeria and India together account for over 121,000 estimated avoidable annual deaths, representing tractable, high-return intervention targets. Methodological caveats are real: episode counts are interpolated between sparse observation years, and GBD modeled estimates carry uncertainty intervals. As a preprint not yet peer-reviewed, these trajectory classifications and country rankings should be treated as provisional. Still, the directional conclusion — that global health equity in child pneumonia survival has made essentially no progress — carries sobering policy weight.
