The biggest survival gains in prostate cancer over the past decade have not come from refining care for early-stage disease — they've come from transforming how advanced, metastatic disease is managed. Understanding where progress is actually happening matters enormously for prioritizing research investment and clinical attention.
Drawing on the KAMU-ON registry, a Japanese multicentre dataset spanning 2010 to 2024, this retrospective cohort study analyzed outcomes across 7,140 patients stratified into localized (n=5,048), nonmetastatic advanced (n=1,128), and metastatic (n=964) prostate cancer. Treatment patterns shifted meaningfully across three diagnostic eras, and multivariable Cox proportional hazards modeling revealed that patients diagnosed in the 2018–2024 period faced a 21% lower hazard of death compared to the earliest cohort (HR 0.79, 95% CI 0.64–0.98). Survival improvement was most visually apparent in the metastatic group, while nonmetastatic advanced disease showed comparatively modest temporal gains. Formal interaction testing between diagnostic era and disease stage did not reach statistical significance (p=0.163), tempering confident stage-specific conclusions.
This finding aligns with the broader oncology narrative: the post-2015 era brought a wave of approved agents for metastatic castration-sensitive and castration-resistant prostate cancer — abiraterone, enzalutamide, apalutamide, darolutamide, and docetaxel combinations — which collectively reshaped the prognosis for metastatic disease. The lackluster temporal improvement in nonmetastatic advanced disease is worth scrutinizing; it may reflect diagnostic heterogeneity or underutilization of newer hormonal agents in that intermediate group. Key limitations include the retrospective, observational design and the Japanese-specific demographic profile, which may limit generalizability to Western populations with different disease biology and treatment access patterns. The non-significant interaction term also means stage-specific conclusions remain exploratory. Overall, this is a solid confirmatory real-world data point rather than a paradigm-shifting finding, but it reinforces that investment in metastatic-stage innovation has translated into measurable population-level survival benefit.
