For cancer patients enduring taxane chemotherapy, peripheral neuropathy — the numbness, tingling, and pain that can persist long after treatment — remains one of the most debilitating and poorly managed side effects. Conventional pharmacology offers limited relief, making integrative approaches like acupuncture increasingly relevant to oncology care teams and patients alike.
This multicenter randomized controlled trial enrolled 48 patients with taxane-induced peripheral neuropathy (TIPN) across three arms: professional acupuncture (12 sessions of combined manual and 2 Hz electroacupuncture over 13 weeks), daily self-applied press-tack needles at specific acupoints, and a wait-list control. The primary endpoint — change in Visual Analogue Scale scores for numbness — showed no statistically significant difference between the acupuncture group and controls (p=0.60). However, patient-reported outcome measures told a more nuanced story: the acupuncture group reported significantly less daily life interference from numbness and tingling (p=0.017) and from pain (p=0.019) compared to controls. Acupuncture also outperformed the press-tack needle group on the numbness/tingling interference measure (p=0.043), and the trial captured data on analgesic medication costs as a secondary outcome.
The divergence between the primary VAS endpoint and patient-reported interference scores is scientifically interesting but also a caution flag. VAS captures symptom intensity, while interference scores reflect functional impact — and the two do not always move in tandem. It is possible acupuncture modestly shifts how patients cope with or perceive neuropathic interference without substantially reducing the raw sensation of numbness. The sample size of 48 is modest, limiting statistical power and generalizability. TIPN is notoriously heterogeneous depending on cumulative taxane dose, patient age, and baseline neuropathy, adding noise to any small trial. This work is best viewed as confirmatory of prior pilot data and supportive of larger definitive trials. For clinical practice, the findings add incremental weight to acupuncture as a low-risk adjunct for functional symptom management in chemotherapy survivors, though prescriptive conclusions remain premature.
