A new review in Nutrients reframes glutathione not merely as a supplement trend but as a mechanistically grounded therapeutic target, synthesizing the current evidence with unusual rigor across preclinical, clinical, and translational data.
Glutathione (GSH) depletion is a consistent metabolic signature in type 2 diabetes, and this review draws on mechanistic studies and clinical trials to evaluate whether restoring GSH levels — either directly through supplementation or via precursor compounds such as N-acetylcysteine and cystine/glycine combinations — can meaningfully alter disease trajectory. Key findings suggest that GSH augmentation is associated with measurable improvements in insulin sensitivity and reductions in oxidative biomarkers, with some evidence pointing toward better microvascular outcomes relevant to diabetic nephropathy and retinopathy. Safety data across populations were notably favorable, with gastrointestinal discomfort as the primary adverse effect and no serious toxicities identified. Importantly, GSH supplementation appeared compatible with standard antidiabetic and antiretroviral drug regimens.
This review arrives at a moment when the supplement landscape around antioxidants has been partly discredited by failed large-scale trials of vitamins C and E in diabetes. Glutathione's intracellular specificity and its role in mitochondrial function distinguish it mechanistically from those earlier approaches. However, the authors are candid that findings remain preliminary and heterogeneous — a critical caveat. No large-scale, dose-standardized randomized controlled trials yet exist.
