For the millions of adults living with both type 2 diabetes and chronic kidney disease, achieving every recommended treatment target simultaneously remains an aspirational benchmark rather than a clinical reality. This analysis raises an important question: does finerenone's protective benefit depend on how well-controlled a patient is at baseline — and the answer carries significant clinical weight.
Drawing on the FIDELITY pooled dataset, which combined individual participant data from the landmark FIDELIO-DKD and FIGARO-DKD phase III randomized controlled trials, this exploratory analysis stratified roughly 13,000 participants by the number of ADA-defined treatment goals met at enrollment. These goals included HbA1c ≤ 53 mmol/mol (7.0%), blood pressure below 130/80 mmHg, LDL cholesterol below 1.81 mmol/L, and use of an SGLT2 inhibitor or GLP-1 receptor agonist. Only 7% of participants had achieved three or more of these targets at baseline, while nearly 30% had met none. A clear gradient emerged: placebo-group composite cardiovascular event rates fell from 6.0 to 3.5 events per 100 patient-years as baseline goal attainment rose from zero to three or more. Crucially, finerenone's relative risk reduction appeared consistent across all subgroups regardless of how many goals had been met.
This finding is clinically meaningful because it reframes finerenone not as a therapy of last resort for poorly controlled patients, but as a broadly applicable mineralocorticoid receptor antagonist whose benefit is additive across the spectrum of metabolic control. The consistency of effect size across subgroups is reassuring, though the exploratory and post-hoc nature of the analysis warrants caution — subgroup analyses can be underpowered to detect true heterogeneity. Additionally, very few participants had met three or more goals, limiting conclusions in the best-controlled stratum. Still, in a landscape where guideline-directed polypharmacy for cardiorenal disease is expanding rapidly alongside SGLT2 inhibitors and GLP-1 agonists, finerenone's apparent baseline-agnostic benefit positions it as a complementary rather than contingent tool in reducing cardiorenal endpoints.
