As fentanyl has displaced heroin across the U.S. drug supply, the clinical calculus for treating opioid use disorder has shifted — higher buprenorphine doses may now be necessary to achieve adequate receptor occupancy and prevent relapse. Whether prescribing patterns have kept pace with that pharmacological reality, and whether they do so equitably across populations, is a critical public health question with direct implications for overdose mortality.
Drawing on over one million buprenorphine treatment episodes from the IQVIA Longitudinal Prescription database — spanning January 2019 through June 2021 — this large cross-sectional analysis found that the dominant maintenance dose category remained the guideline-consistent 16–24 mg range (62.7%), while a substantial 34.5% of episodes involved sub-therapeutic doses below 16 mg. Only 2.8% of episodes exceeded 24 mg daily, the threshold increasingly discussed as potentially more effective against fentanyl-involved use disorder. Among 543,326 individuals with a mean age of roughly 42, county-level social vulnerability — measured via the Minority Health Index — was significantly associated with dose category, suggesting that geography and structural disadvantage shape treatment intensity independently of clinical need.
This analysis arrives at a moment when addiction medicine specialists are actively debating upward dose revisions to buprenorphine guidelines. The finding that more than one-third of treatment episodes fall below even the standard target range is clinically concerning; buprenorphine's ceiling effect on respiratory depression gives it a wide safety margin, making under-dosing a larger risk than over-dosing in most outpatient settings. The geographic disparity dimension is perhaps the most actionable signal here: if social vulnerability predicts lower doses, then structural barriers — formulary restrictions, prescriber conservatism, pharmacy availability — may be compounding biological vulnerability in already high-risk communities. Limitations include the cross-sectional design, reliance on pharmacy claims rather than clinical outcomes, and a study window that predates the full acceleration of illicitly manufactured fentanyl analogs. Still, with over half a million individuals represented, this is among the larger real-world dose distribution analyses published to date, making it a meaningful benchmark for policy and prescriber education efforts.
