Most adults think of daylight saving time as a minor inconvenience, but mounting evidence suggests the biannual clock shift does measurable biological harm — and a new analysis in JAMA adds a striking ocular dimension to that story. Disrupting the body's circadian rhythm even by a single hour may not be trivial when it comes to the delicate microvasculature of the retina.

The JAMA-published study found a statistically significant association between daylight saving time transitions and increased incidence of retinal vascular diseases — conditions that include retinal vein occlusion, retinal artery occlusion, and related ischemic events that can threaten vision. The analysis drew on population-level data to compare disease rates in periods surrounding the spring and autumn clock changes, identifying an elevated risk window proximate to the time-shift events. The retinal vasculature, supplied by some of the smallest and most pressure-sensitive blood vessels in the body, appears particularly vulnerable to the hemodynamic and inflammatory perturbations that circadian misalignment can provoke.

This finding sits within a growing body of research documenting that clock-shift periods correlate with spikes in cardiovascular events, including myocardial infarction and ischemic stroke. The retinal connection is biologically plausible: the eye's vascular network shares endothelial vulnerabilities with coronary and cerebral circulation, and circadian misalignment is known to transiently elevate cortisol, blood pressure, and platelet aggregability — a constellation that raises thrombotic risk. What makes this work noteworthy is that it extends the known harm profile of DST into ophthalmology, a domain not previously well-studied in this context. Key limitations worth acknowledging include the observational design, which cannot establish causation, potential confounding from seasonal variation in physical activity or UV exposure, and uncertainty about whether effect sizes are clinically meaningful at the individual level.
