Precision medicine for aging is moving beyond one-size-fits-all recommendations, and a new feasibility trial is putting that ambition to a rigorous test. The PROMETHEUS study, a semi-finalist in the XPRIZE Healthspan competition, represents one of the more structurally sophisticated attempts to combine biological phenotyping with adaptive, multimodal intervention in a single protocol — a design that could influence how future healthspan trials are constructed.
The eight-week trial enrolled 20 middle-aged to older adults and deployed a layered intervention architecture. All participants received a foundational protocol: sleep and dietary guidance, supervised exercise incorporating exergaming for dual cognitive-physical training, motivational interviewing, and cognitive behavioral therapy. On top of this, supplementation with whey protein, creatine, and fucoidan was standardized across the cohort. A second personalized layer — the augmented intervention — was allocated based on individually assigned "gerotypes," biological aging profiles derived from muscle mass, peak oxygen uptake, and cognitive performance. Depending on these baseline profiles, participants received urolithin A, nicotinamide mononucleotide, or a multivitamin-mineral complex. At the trial's midpoint, protocols were dynamically adjusted; participants showing inadequate cognitive response could receive ergothioneine supplementation, adding an adaptive dose-escalation dimension rarely seen in this type of exploratory design. Primary outcomes span muscle strength and mass, immune function, cognitive performance, and feasibility metrics including adherence.
Several features make this protocol methodologically noteworthy, even as its small sample size of 20 limits statistical power considerably. The gerotype framework — stratifying participants by biological rather than chronological age — mirrors growing scientific consensus that aging is deeply heterogeneous. The use of ergothioneine as a contingency supplement is particularly forward-looking, given emerging evidence around its role in cellular antioxidant defense. The exergaming component addresses the frequently neglected cognitive dimension of physical training. As a feasibility study, PROMETHEUS is designed to test logistics and tolerability rather than confirm efficacy, so its findings will be hypothesis-generating rather than conclusive. Its true value lies in whether the gerotype-personalization model proves operationally viable at scale — a prerequisite for the larger trials this approach ultimately demands.
