The landscape of obesity pharmacotherapy has shifted so rapidly in the last five years that clinicians and patients face a genuinely difficult comparative question: which agent, and at what trade-off? A comprehensive head-to-head evidence synthesis now offers the clearest population-level answer yet, ranking 19 drugs across 24 outcome measures in a single analytical framework.

Drawing on 262 randomized controlled trials enrolling nearly 100,000 participants — with follow-up windows ranging from 12 to 172 weeks — this BMJ network meta-analysis used both frequentist random-effects models and Bayesian dose-response modeling to estimate one-year weight reduction against a lifestyle-modification comparator. Tirzepatide (a dual GIP/GLP-1 receptor agonist) and the investigational cagrilintide-semaglutide combination each produced approximately 14.8–14.9% body weight reduction with moderate-to-high certainty evidence. Oral semaglutide achieved roughly 10.9% reduction, followed closely by the oral small-molecule GLP-1 agonist orforglipron at 9.9%, subcutaneous semaglutide at 9.8%, and the older combination phentermine-topiramate at 8.1%. Three pipeline agents — ecnoglutide, mazdutide, and retatrutide — showed 13–15% reductions, though confidence in those estimates is currently low.

Several dimensions make this analysis especially valuable beyond weight-loss rankings alone. Network meta-analyses of this scale can reveal comparative safety signals that individual trials are underpowered to detect, and the inclusion of GRADE certainty ratings allows clinicians to distinguish well-replicated estimates from preliminary ones. Still, important caveats apply: most trials were industry-sponsored, follow-up rarely exceeded three years, and discontinuation rates — which substantially affect real-world effectiveness — vary widely across agents. The absence of hard cardiovascular endpoint data for several newer molecules is a meaningful gap, given that the SELECT trial elevated cardiovascular risk reduction to a co-primary value proposition for semaglutide. For health-conscious adults and prescribers navigating this rapidly expanding drug class, this synthesis represents a genuinely paradigm-organizing contribution rather than an incremental update — the most rigorous comparative map of the obesity pharmacotherapy field to date.
