The intersection of gender-based violence and infectious disease burden rarely receives quantified, country-level scrutiny — yet understanding how intimate partner violence amplifies HIV vulnerability in women may be essential to reversing one of the few areas where global health progress is genuinely at risk of backsliding. This landmark analysis from the Global Burden of Disease 2023 framework puts hard numbers to a relationship long understood in principle but rarely modeled at scale.
Drawing on data from 204 countries and territories spanning 1990 to 2023 — incorporating national HIV programme reports, population-based serosurveys, clinical records, and vital registration data — the study estimated annual HIV incidence, prevalence, and mortality disaggregated by age and sex. A novel methodological layer combined time-series IPV prevalence estimates with effect-size data to generate population attributable fractions, quantifying how much of the HIV burden in women is statistically linked to intimate partner violence exposure. The analysis then applied a non-parametric stochastic frontier model to forecast how declining development assistance for health could reverse hard-won progress through 2030, modeling the downstream consequences of funding reductions on antiretroviral coverage and mortality trajectories.
This study is arguably paradigm-shifting in its scope, not because the IPV-HIV relationship is new — epidemiological evidence has supported this link for decades — but because generating country-level attributable fractions at this resolution creates a policy-actionable evidence base that previously did not exist. The forecasting component is particularly timely given recent geopolitical shifts affecting global health financing. Key limitations include the observational and modeled nature of attributable fraction estimates, which cannot establish causality, and the inherent uncertainty in stochastic forecasting models. Nevertheless, the GBD framework is among the most methodologically rigorous global epidemiological tools available. For health-conscious adults tracking longevity and population health trajectories, this work underscores that infectious disease burden remains inseparable from social determinants — and that financial infrastructure sustaining antiretroviral programs is itself a determinant of survival at population scale.
