For the 39 million people living with HIV globally, the trajectory toward elimination has never been more fragile — and this landmark analysis reveals two underappreciated threats: the persistent role of intimate partner violence in driving female HIV acquisition, and the quantified mortality consequences of development aid reductions. Understanding how structural violence intersects with infectious disease risk reframes HIV prevention as inseparable from gender equity.
Drawing on the Global Burden of Disease 2023 framework, researchers reconstructed HIV incidence, prevalence, and mortality across 204 countries and territories from 1990 through 2023, stratified by age and sex. By integrating time-series estimates of intimate partner violence prevalence with established effect sizes, the team calculated population-attributable fractions — the proportion of HIV burden statistically attributable to IPV exposure. Separately, a non-parametric stochastic frontier model was used to project how reductions in development assistance for health could shift antiretroviral coverage and, by extension, mortality curves through 2030. The dual-threat analysis is methodologically ambitious, combining epidemiological surveillance data, serosurveys, and clinical registries at unprecedented geographic granularity.
This study's significance lies partly in its timing. The PEPFAR funding debates and broader development assistance contractions occurring since 2023 are not abstract policy concerns — they translate directly into modeled mortality increases that this analysis attempts to quantify. The IPV-attributable fraction finding reinforces a body of evidence, including prior GBD cycles and longitudinal African cohort studies, showing that violence exposure elevates HIV risk through multiple pathways: disrupted condom negotiation, reduced healthcare-seeking, and biological susceptibility linked to traumatic injury. A critical limitation is that population-attributable fractions depend heavily on IPV prevalence estimates, which remain systematically underreported in low-income settings, potentially understating the true attributable burden. The forecast modeling, while methodologically rigorous, rests on assumptions about funding-program linkages that involve policy uncertainty. Nonetheless, as a large-scale, multi-decade, multi-country analysis published in The Lancet HIV, this work carries substantial evidentiary weight and should inform both prevention programming and health economics for the remainder of this decade.
