A benchtop NMR spectrometer operating at 80 MHz — far less powerful than laboratory-grade 600 MHz instruments — successfully quantified key cardiovascular biomarkers in 336 individuals using just 32 scans completed in 2 minutes 8 seconds. Triglycerides and glucose achieved the highest accuracy (R² ≥95%, CV ≤5.2%), apolipoprotein B performed moderately (R² =74%, CV =7.4%), while LDL cholesterol proved most difficult (R² =72%, CV =10.1%). Critically, epidemiological associations between these biomarkers and sex, obesity, glycemia, and blood pressure were faithfully reproduced by the benchtop method.
This is a genuinely practical advance worth watching. LDL and apolipoprotein B are causally linked to atherosclerotic cardiovascular disease — the world's leading killer — yet accurate lipid testing remains inaccessible in low-resource clinics, mobile screening units, and remote settings where cold-chain laboratory logistics are prohibitive. A portable, affordable NMR device that returns a near-complete cardiometabolic panel in under three minutes could meaningfully shift early detection capacity globally. That said, LDL accuracy at R² =72% falls below clinical-grade thresholds typically demanded for treatment decisions; whether further algorithmic refinement closes this gap remains to be demonstrated in larger, more diverse cohorts. The 336-person sample is modest, and real-world variables like sample degradation and operator variability aren't fully addressed. As a preprint not yet peer-reviewed, these results may be revised substantially. Still, the directional finding — that affordable benchtop NMR is feasible for lipoprotein screening — is incrementally significant and clinically promising.
