Skin disease affects nearly five billion people annually, yet the global workforce trained to treat it remains dramatically unequal across income levels — a gap with real consequences for disability, disfigurement, and quality of life. Understanding the true scale of that inequality, country by country, has been hampered by sparse data. This large cross-sectional survey begins to fill that void with unusually broad geographic reach.
Conducted across 194 WHO member states and three additional geographic areas, the study achieved an exceptional response rate exceeding 80%, with replies from 158 countries representing 97% of the global population. Using a 47-question Delphi-derived instrument distributed to dermatology leaders between August 2024 and October 2025, investigators mapped dermatologist density per 100,000 people, training program availability, workforce distribution patterns, perceived care access, and the role of non-dermatologist providers. Results were stratified by World Bank income level — low, lower-middle, upper-middle, and high — as well as by WHO region, allowing direct comparisons of how wealth correlates with specialist availability.
This study stands out in health-systems research for its near-universal country coverage, a methodological achievement that most global health surveys rarely approach. The Delphi-derived instrument adds rigor by grounding the questionnaire in expert consensus. That said, reliance on country-level dermatology leaders as respondents introduces potential reporting bias, particularly where accurate workforce registries are absent. The study is also cross-sectional, capturing a snapshot rather than trends over time. Practically, these findings matter because skin conditions — ranging from eczema and psoriasis to infectious diseases and skin cancers — disproportionately burden low-income populations who have the least access to trained specialists. Quantifying the dermatologist density gap is the essential first step toward targeted workforce expansion, task-sharing frameworks, and teledermatology investment. For health systems researchers and policymakers alike, this dataset offers a credible baseline from which to measure future progress.
