Osteoporosis quietly fractures lives before most people know they have it — and new survey data suggest that ignorance about the condition is far more widespread than the disease itself. Among a population group arguably most at risk, foundational knowledge about prevention remains dangerously thin, raising real questions about whether current public health messaging is reaching those who need it most.
A validated questionnaire administered to 1,432 Australians aged 50 and older (mean age 64.3 years) revealed an average osteoporosis knowledge score of just 29.5 out of 100 — a striking deficit for a condition that affects roughly one in three women and one in five men over 50 globally. Knowledge was most deficient around balance-improving exercise (mean score 11.1/100), a critical but underappreciated pillar of fracture prevention. Calcium sources scored highest at 35.0/100, suggesting even the best-retained information barely clears a third of what was tested. Multivariable regression identified being male (OR 2.40), lacking a family history of osteoporosis (OR 1.79), lower educational attainment (OR 1.38), and advancing age (OR 1.02 per year) as independent predictors of falling in the lowest knowledge tertile.
These findings sit within a well-documented paradox: osteoporosis is simultaneously one of the most preventable and most under-recognized chronic conditions in aging populations. While calcium and vitamin D supplementation dominate public discourse, the role of resistance and balance training in fracture risk reduction receives far less attention — a gap this data quantifies. The strong male knowledge deficit is particularly concerning given that men with osteoporotic fractures face disproportionately higher mortality than women. The study's limitations include its cross-sectional, self-selected online design, which may systematically overestimate knowledge in the broader population. The sample skewed toward engaged, health-literate volunteers, meaning real-world gaps could be even larger. Nonetheless, the granular identification of at-risk demographic subgroups provides actionable targets for tailored health promotion — especially for men and those without personal family exposure to the disease.
