Brazilian Ministry of Health surveillance data covering January 2023 through June 2025 reveals a stark respiratory virus burden shift: RSV mortality among children under 5 reached 60 per 100,000 in Q2 2025—a 43% jump from Q2 2024. Combined mortality from parainfluenza, adenovirus, rhinovirus, bocavirus, and metapneumovirus doubled to 40 per 100,000 in the same pediatric cohort. Influenza mortality tripled among adults 65 and older over the same interval. Meanwhile, SARS-CoV-2 deaths in the elderly collapsed from 150 to just 25 per 100,000 between Q1 2023 and Q2 2025, reflecting high population immunity.

This epidemiological reshaping echoes a pattern observed globally post-pandemic: as COVID-19 immunity accumulates through infection and vaccination, RSV and influenza reclaim their historic roles as the dominant killers of the very young and very old. The near-disappearance of these viruses during 2020–2022 lockdowns may have deepened immunological naivety in cohorts born during that period, potentially amplifying RSV severity in toddlers now. The arrival of maternal RSV vaccines and infant RSV monoclonal antibodies (nirsevimab) makes the policy implication direct: Brazil's mortality spike may be partially preventable with accelerated rollout. Important caveats apply—this is a preprint not yet peer-reviewed, causation requires confirmation, and multinomial imputation was used for laboratory-unconfirmed deaths, introducing modeling uncertainty. Still, the scale of the signal makes this epidemiologically significant and warrants urgent public health attention.
