In 119 rheumatology outpatients with high cardiometabolic burden (mean BMI 34.9 kg/m², 96.6% diabetic), GLP-1 receptor agonist therapy produced progressive weight loss of 4.0 kg, 4.5 kg, and 7.7 kg at 3, 6, and 12 months respectively—an 8.2% reduction at one year. HbA1c fell by roughly 0.9% at each interval, fasting glucose dropped up to 24.8 mg/dL, LDL-cholesterol declined 12.2 mg/dL, and triglycerides fell 21.7 mg/dL. Critically, erythrocyte sedimentation rate dropped significantly at 12 months in the full cohort, and both ESR and CRP declined significantly in the 46-patient chronic inflammatory arthritis subgroup.
The inflammation signal is the most provocative element here. GLP-1 receptors are expressed on immune cells including macrophages and T-cells, and preclinical data have long suggested direct anti-inflammatory pathways beyond weight loss. This real-world cohort—while retrospective, single-centre, and heavily diabetic—adds clinical texture to that mechanistic hypothesis. The nearly exclusive diabetes prevalence (96.6%) limits generalizability to the broader rheumatology population, and without a matched control arm, separating GLP-1-specific anti-inflammatory effects from weight-loss-mediated reductions in systemic inflammation is impossible. That CRP trended but didn't reach significance (p=0.076) while ESR did suggests either a power limitation or differential sensitivity of these markers. The 6.7% discontinuation rate and low rate of serious adverse events are reassuring. This is confirmatory-incremental work that strengthens the rationale for prospective, controlled trials examining GLP-1RAs as dual metabolic-inflammatory agents in rheumatology.
