Among 5,895 MI survivors tracked for nearly 16 years across the Nurses' Health Study and Health Professionals Follow-Up Study, adherence to any of eight established healthy dietary patterns was consistently associated with lower total mortality. Hazard ratios for highest versus lowest quintile ranged from 0.66 (Alternate Mediterranean Diet Score) to 0.77 (DASH and anti-inflammatory pattern), representing 23–34% mortality reductions. Crucially, improving diet quality after MI — measured by Alternate Healthy Eating Index gains — yielded an additional 14% mortality reduction (HR 0.86), while declining adherence predicted worse outcomes.

What distinguishes this analysis is its cross-pattern consistency: eight conceptually distinct dietary frameworks — spanning Mediterranean, plant-based, anti-inflammatory, and insulin-modulating indices — converged on the same protective signal. This near-unanimity is statistically rare and clinically reassuring, suggesting the underlying mechanism is dietary quality broadly construed, not any single food group or biomarker pathway. The insulin-resistance and hyperinsulinemia indices showing comparable benefit to Mediterranean scoring is particularly noteworthy, implicating metabolic load as a key post-MI mortality driver.

Limitations are real: the cohort skews educated, predominantly white healthcare professionals, limiting generalizability. Observational design cannot exclude residual confounding, and self-reported food frequency questionnaires introduce measurement error. Still, with nearly 3,900 deaths across a 15-year window, statistical power is robust. For clinicians and patients alike, the finding that diet improvement post-MI still confers survival benefit — regardless of prior habits — is the most actionable and underappreciated result here.
