Drawing on US population-level survey data spanning 2011 to 2024, the study tracked three distinct drinking patterns among pregnant women: current alcohol use, binge drinking episodes, and heavy drinking. The analysis examined not just prevalence trends over time but also demographic and behavioral correlates — meaning researchers identified which subpopulations showed the most pronounced changes, and in which direction. The granularity of distinguishing binge from heavy drinking is methodologically important, as these patterns carry different fetal risk profiles and likely require different intervention strategies.

This work arrives at a moment when broader US alcohol consumption trends have been paradoxical: overall drinking rose during the COVID-19 pandemic, while younger cohorts have trended toward reduced use. How those countervailing forces intersect with pregnancy is precisely what surveillance of this kind can disentangle. The findings carry real clinical weight — no safe threshold for prenatal alcohol exposure has been established, and even moderate intake has been associated with subtle cognitive and behavioral effects in offspring. A critical limitation of survey-based designs is self-report bias, which likely underestimates true prevalence given social desirability pressures around drinking during pregnancy. The study is observational and cannot assess causal drivers of any trends. Still, 13 years of consistently collected national data provide a robustness that single-point estimates cannot. For clinicians, the correlate analysis may be the most actionable output, potentially identifying high-risk windows or demographic groups where screening intensity should be concentrated.
