Tuberculosis remains one of the most consequential infectious disease challenges of the modern era, and a comprehensive new accounting of its toll forces a reckoning with how far the world still is from elimination targets — targets now further threatened by retreating global health investment. For health-conscious adults tracking population health trajectories, this analysis represents the most granular longitudinal picture of TB's reach yet assembled.

Drawing on the Global Burden of Disease Study 2023 framework, researchers modeled TB incidence, prevalence, mortality, and disability-adjusted life-years (DALYs) across 204 countries and territories from 1990 through 2023. The analysis stratified outcomes by HIV co-infection status and drug-resistance profiles — including multidrug-resistant TB (MDR-TB) — using ensemble mortality modeling integrated with vital registration, surveillance, verbal autopsy, and minimally invasive tissue sampling data. From an analytical standpoint, this is a landmark confirmatory and baseline-setting study rather than a paradigm-shifting mechanistic finding — but its epidemiological scope and policy relevance are substantial.
