Every minute between a serious vehicle crash and first medical contact shapes survival odds — a reality that makes the slow adoption of existing crash-alert technology one of the more frustrating gaps in emergency medicine. Automatic and Advanced Automatic Crash Notification (ACN/AACN) systems can detect collision severity, estimate injury probability, and trigger an immediate 911 alert before any occupant is conscious enough to call for help, yet these systems remain absent from much of the vehicle fleet and largely disconnected from the emergency dispatch infrastructure that would act on their data.
This review in the Journal of the American College of Surgeons synthesizes the literature on ACN and AACN outcomes, tracing the technology's origins to the NHTSA's 1995 Automatic Collision Notification project. AACN goes beyond simple crash detection by transmitting vehicle telematics — delta-v (change in velocity), rollover status, airbag deployment, and seat-belt use — to public safety answering points (PSAPs). These data points enable dispatchers to pre-activate trauma teams and route patients to appropriate trauma centers before the ambulance arrives, compressing the critical window between injury and definitive care. The broader context here is the well-established 'golden hour' framework in trauma surgery, where outcomes deteriorate sharply with delayed intervention.
