For families navigating a rare neurodevelopmental disorder, fragmented and inconsistent medical guidance can be as debilitating as the condition itself. Kleefstra syndrome — caused by disruption of the EHMT1 gene, which encodes a histone methyltransferase critical to chromatin regulation and neurodevelopment — has until now lacked any unified international framework for clinical care, leaving patients and clinicians in different countries working from scattered, heterogeneous knowledge bases.

An international consortium of 43 participants — spanning clinical geneticists, neurologists, developmental specialists, and patient representatives from 15 countries — convened to produce the first comprehensive, evidence-based clinical guideline for Kleefstra syndrome (KLEFS1). The resulting document delivers 66 discrete clinical recommendations covering the syndrome's multisystem involvement, from neurocognitive and behavioral management to cardiac, renal, and endocrine surveillance. The process employed formal consensus methodology to ensure broad agreement across diverse healthcare contexts and to establish a reproducible minimum standard of care applicable globally.

From a broader clinical genetics perspective, this guideline addresses a persistent structural gap in rare disease management: the inverse relationship between disease rarity and care consistency. EHMT1 disruptions — whether via deletion at 9q34.3 or point mutations — affect chromatin remodeling pathways that are increasingly implicated across a spectrum of neurodevelopmental conditions, making KLEFS1 an important model disorder. The consortium's methodological approach, explicitly designed to be generalizable to other rare disorders, may carry significance beyond Kleefstra syndrome itself. Key limitations are inherent to the territory: rare disease guidelines inevitably rest on lower-evidence clinical data, smaller cohort studies, and expert opinion rather than large randomized trials. The 66 recommendations will require prospective validation as registry data matures. Nevertheless, for clinicians, caregivers, and health systems currently managing KLEFS1 without a reference framework, this guideline represents a meaningful and overdue consolidation of expert knowledge into actionable, standardized care.
