For patients with BRAF-mutant advanced melanoma, the sequence and timing of treatment matters enormously — and a real-time molecular signal may now offer a more rational basis for switching between two powerful but mechanistically distinct therapies than clinical judgment alone. The CAcTUS trial addresses a genuine clinical dilemma: checkpoint immunotherapy generally confers superior long-term survival, yet targeted BRAF/MEK inhibitor therapy can produce faster, deeper responses that may help poor-prognosis patients reach a more immunologically receptive state.

The randomized phase II feasibility trial enrolled 21 patients across two arms. A control arm of ten patients received standard investigator-selected targeted therapy (TT) or checkpoint immunotherapy (CPI), switching upon disease progression. The experimental arm of eleven patients commenced TT and transitioned to CPI once circulating tumor DNA — specifically BRAF variant allele frequency (VAF) — dropped by 80% or more. Both primary feasibility endpoints were met: critical ctDNA results were delivered within seven days in 100% of cases, and every TT-treated patient achieved the 80% VAF reduction threshold. Post-hoc analyses also found that cytokine and chemokine profiles at ctDNA nadir appeared more favorable for subsequent immune engagement. Longitudinal ctDNA tracking further suggested it can serve as an early indicator of CPI response and that re-challenge with TT after CPI progression could elicit renewed molecular responses.

While the findings are compelling in concept, the trial's 21-patient cohort limits any definitive conclusions about survival benefit or the generalizability of the ctDNA-guided switching strategy. This is a feasibility study — it was designed to determine whether the logistical and biological framework is viable, not to power survival comparisons. Still, the mechanistic rationale is sound: BRAF inhibition may transiently reshape the tumor immune microenvironment, and catching that window with immunotherapy could compound long-term benefit. The trial's real contribution is demonstrating that real-time liquid biopsy data can be operationalized within a clinically actionable seven-day window. A larger, adequately powered randomized trial is now the necessary next step to determine whether ctDNA-guided sequencing translates into measurable survival gains.
