For the roughly 9–24% of women undergoing IVF who are classified as poor ovarian responders, every retrieved oocyte carries outsized weight. If a low-cost, low-risk adjunct could meaningfully shift that number, it would matter enormously to patients who have often already exhausted standard pharmacological options — and this large retrospective study from a specialized Chinese fertility center suggests acupuncture may be one such adjunct.
The cohort enrolled 2,133 women with poor ovarian response (POR) treated between January 2020 and October 2024, of whom 887 received acupuncture — typically three sessions per week spanning ovarian stimulation through embryo transfer — while 1,246 served as controls. The primary finding was a notably higher median oocyte yield in the acupuncture arm (4.0 versus 3.0), with a standardized mean difference of 1.7 (95% CI: 1.53–1.95, p < 0.001). Secondary endpoints — clinical pregnancy rate and live birth rate — directionally favored the acupuncture group as well, with subgroup analysis suggesting a dose-response pattern: more than four sessions conferred greater benefit than four or fewer.
The result is biologically plausible. Acupuncture is hypothesized to modulate ovarian blood flow via autonomic nervous system pathways and to influence gonadotropin sensitivity through neuroendocrine signaling, potentially improving follicular microenvironment. However, the study's retrospective, non-randomized design is a critical limitation: patients self-selected into the acupuncture group, introducing unmeasured confounding despite multivariable adjustment for FSH, AMH, and prior failed cycles. Women who voluntarily pursue acupuncture may also differ in health behaviors, adherence, and motivation in ways that inflate apparent benefit. The single-center setting further limits generalizability. This finding should be characterized as hypothesis-generating rather than practice-changing — a well-powered randomized controlled trial is the necessary next step before acupuncture can be confidently integrated into POR management protocols.
