A comprehensive review in The Lancet Diabetes & Endocrinology reframes GLP-1-based therapies as genuinely systemic medicines — with measurable effects spanning metabolic, cardiovascular, neuropsychiatric, and structural disease categories that persist even when weight loss is controlled for. This is a meaningful shift in how clinicians and researchers understand the therapeutic ceiling of this drug class.
The review synthesizes evidence from randomized controlled trials and high-quality meta-analyses covering both approved agents — including liraglutide, oral and subcutaneous semaglutide, phentermine-topiramate, and naltrexone-bupropion — and late-stage investigational multiagonists such as tirzepatide, retatrutide, survodutide, mazdutide, cagrilintide-semaglutide, and amycretin. Across 14 distinct comorbid conditions — including metabolic dysfunction-associated steatotic liver disease, chronic kidney disease, heart failure, obstructive sleep apnea, polycystic ovary syndrome (now termed polyendocrine metabolic ovarian syndrome), osteoarthritis, depression, binge-eating disorder, substance use disorders, and neurodegenerative disease — GLP-1 receptor agonists and multiagonist therapies demonstrated beneficial signals.
