Among 3,612 UK Biobank participants with type 2 diabetes followed for up to 12.1 years, those in the highest quartile of Mediterranean Lifestyle Index (MEDLIFE) adherence showed a 27% reduction in major adverse cardiovascular events (HR 0.73), 26% lower all-cause mortality (HR 0.74), and a striking 51% reduction in cardiovascular mortality (HR 0.49) compared to the lowest quartile. All three MEDLIFE blocks — food consumption, dietary habits, and physical activity/social habits — independently contributed to these associations.
What elevates this finding beyond the well-worn Mediterranean diet literature is the inclusion of lifestyle behaviors beyond food: rest, physical activity, and social conviviality are scored alongside dietary patterns. This composite framing matters because type 2 diabetes management has long focused on pharmacology and glycemic targets, while behavioral architecture receives less clinical attention. The 51% CVD mortality reduction is particularly striking and exceeds the magnitude typically seen with dietary interventions alone, suggesting synergistic effects across lifestyle domains.
Limitations deserve acknowledgment: UK Biobank participants skew healthier and more educated than the general population, introducing healthy volunteer bias. MEDLIFE adherence was assessed at baseline only, missing lifestyle drift over a decade. Observational design precludes causal inference, and residual confounding by socioeconomic status — closely correlated with both lifestyle quality and cardiovascular outcomes — cannot be fully excluded. Still, with a large cohort, long follow-up, and robust sensitivity analyses, this is a substantively confirmatory and clinically actionable result for diabetes care teams.
