Emerging evidence across multiple organ systems suggests these medications — particularly GLP-1 receptor agonists and newer multiagonist compounds — may fundamentally alter the trajectory of a dozen or more serious chronic diseases, independent of how much weight a patient loses. This recasting of obesity as a 'gateway disease' rather than a lifestyle failure has profound implications for how clinicians prioritize treatment and how payers justify coverage.

This Lancet Diabetes & Endocrinology review synthesizes data from randomized controlled trials and high-quality meta-analyses evaluating approved and late-stage obesity medications: phentermine-topiramate, naltrexone-bupropion, GLP-1 receptor agonists including liraglutide and both subcutaneous and oral semaglutide, and next-generation multiagonists such as tirzepatide, retatrutide, survodutide, mazdutide, cagrilintide-semaglutide, and amycretin. Conditions assessed span type 2 diabetes, metabolic dysfunction-associated steatotic liver disease (MASLD), chronic kidney disease, heart failure, cardiovascular disease, obstructive sleep apnea, polycystic ovary syndrome, osteoarthritis, muscle mass preservation, depression, food cravings, binge-eating disorder, substance use disorders, and neurodegenerative disease — a striking breadth that no single drug class has previously addressed in one systematic review.

What elevates this analysis above prior literature is the explicit focus on weight-loss-independent mechanisms. GLP-1 receptors are expressed throughout the brain, gut, heart, and kidneys, and the review accumulates evidence that direct receptor signaling — not merely downstream adiposity reduction — drives meaningful improvements in cardiovascular outcomes, neuropsychiatric symptoms, and possibly neurodegeneration.
