The PNAS study investigates the clinical behavior of Klebsiella pneumoniae liver abscess (KPLA) with a focus on two converging problems: antibiotic tolerance and the limited therapeutic benefit derived from percutaneous or surgical drainage. Antibiotic tolerance — distinct from resistance in that bacteria survive drug exposure without acquiring resistance genes — appears to allow K. pneumoniae to persist through otherwise adequate antibiotic courses. The research characterizes the mechanistic and clinical dimensions of this tolerance phenotype and examines why drainage, a cornerstone intervention, produces inconsistent outcomes in KPLA compared to abscesses caused by other organisms.
This finding situates KPLA in a particularly difficult clinical niche. Antibiotic-tolerant bacteria are notoriously difficult to detect using standard susceptibility testing, meaning cultures may report sensitivity while the organism behaviorally resists eradication.
