A lung disease that can kill workers within years of first exposure — not decades — is quietly spreading through the countertop fabrication industry, and the safety measures most workplaces rely on are failing to stop it. This 2025 American Thoracic Society workshop report synthesizes global evidence on artificial stone-associated silicosis (AS silicosis), a condition that now demands urgent clinical and public health attention precisely because it strikes fast, hard, and young.

The report documents that engineered stone countertops — the quartz-composite surfaces now ubiquitous in kitchens and bathrooms — contain far higher concentrations of crystalline silica than natural stone, often exceeding 90% silica content by weight. When cut, ground, or polished, these materials generate respirable crystalline silica dust at concentrations that routinely surpass established occupational exposure limits even when wet cutting, local exhaust ventilation, and respiratory protective equipment are in use. Workers develop progressive massive fibrosis — the most severe form of silicosis — at unusually short latencies, often within five to ten years of first exposure, with concurrent elevated rates of autoimmune disease and susceptibility to infection. The affected population is disproportionately young, male, and employed in informal or precarious labor arrangements.

Conventional silicosis has been recognized for over a century, but AS silicosis represents a genuinely distinct and more aggressive phenotype that existing occupational health frameworks were not designed to handle.
