A lethal, drug-resistant fungal pathogen is quietly expanding its foothold across European hospitals — and the resistance data now emerging from a comprehensive systematic review should concern anyone tracking infectious disease threats to vulnerable populations. With mortality rates between 30% and 60%, Candida auris represents one of the more alarming nosocomial pathogens of the past decade, and its spread through healthcare settings is proving difficult to contain.

This PRISMA-compliant systematic review analyzed 2,191 clinical isolates from 16 European countries, drawing on 91 studies indexed across four major databases through January 2025. Spain dominated the dataset with 886 isolates (40.4%), followed by Italy with 553 (25.2%), Greece with 214 (9.8%), the United Kingdom with 182 (8.3%), and Russia with 108 (4.9%) — together accounting for nearly 89% of all reported European cases. The pathogen's survival advantages are formidable: it persists on surfaces, withstands hospital-grade disinfectants, tolerates desiccation, and transmits readily via direct and indirect contact, making environmental decontamination particularly difficult in clinical settings.

What distinguishes this review from previous regional reports is its breadth and recency. C. auris was first identified in 2009 in Japan and remained largely confined to Asia, the Middle East, and South Africa for several years. Its European emergence has accelerated since roughly 2016, with the geographic spread now clearly documented. The antifungal resistance profile is especially concerning: many isolates demonstrate intrinsic resistance to fluconazole, reduced susceptibility to amphotericin B, and an increasing proportion show resistance to echinocandins — the current first-line treatment — leaving clinicians with narrowing therapeutic options. This systematic review is a significant contribution to surveillance science, though its retrospective observational design means case ascertainment is highly dependent on local diagnostic capacity, likely underrepresenting countries with less robust clinical mycology infrastructure. The concentration of cases in southern and eastern Europe likely reflects both genuine epidemiological burden and differential reporting intensity. For health-conscious adults, the near-term implication is institutional: C. auris primarily threatens immunocompromised, critically ill, or post-surgical patients in healthcare settings rather than healthy community members.
