Fatty liver disease has quietly become one of the most prevalent chronic liver conditions worldwide, yet it remains systematically under-recognized in primary care settings. The renaming from nonalcoholic fatty liver disease (NAFLD) to metabolic dysfunction-associated steatotic liver disease (MASLD) is not merely semantic — it reframes the condition as fundamentally cardiovascular-metabolic in origin, demanding a more proactive management posture from clinicians who may previously have watched and waited.

The updated clinical framework defines MASLD as hepatic fat accumulation accompanied by at least one cardiometabolic risk factor — such as elevated BMI, dysglycemia, hypertension, or dyslipidemia — in the absence of significant alcohol use or secondary causes. Risk stratification now follows a deliberate two-step noninvasive pathway: initial assessment using the Fibrosis-4 (FIB-4) index, a simple calculation derived from age, liver enzymes, and platelet count, followed by imaging-based confirmation via vibration-controlled transient elastography or ultrasound elastography for those with indeterminate or elevated FIB-4 scores. Critically, coexisting diabetes and obesity substantially elevate long-term risk for cirrhosis and hepatocellular carcinoma. Two pharmacological agents — semaglutide, a GLP-1 receptor agonist already widely used in metabolic disease, and resmetirom, a thyroid hormone receptor-beta agonist specifically targeting hepatic lipid metabolism — have now received FDA approval for MASLD treatment.

This guidance arrives at a meaningful inflection point. Resmetirom's approval in 2024 marked the first liver-directed pharmacotherapy specifically indicated for metabolic steatohepatitis with fibrosis, a milestone roughly two decades in the making. Semaglutide's inclusion signals that cardiometabolic and hepatic treatment goals are increasingly inseparable. The FIB-4-first triage model is a pragmatic advance, enabling primary care physicians to identify high-risk patients without reflexively ordering specialist imaging. Limitations here are real: the guidance emerges from a review article rather than a primary trial, and real-world implementation of two-step elastography testing remains inconsistent across healthcare systems. For most patients, the cornerstone intervention remains lifestyle modification — particularly a minimum 5% body weight reduction — which can meaningfully reverse early-stage steatosis. This guidance is confirmatory in its lifestyle recommendations but genuinely incremental in codifying a structured, drug-inclusive management algorithm for primary care.
