For the roughly half-million patients worldwide who undergo surgical resection for non-small cell lung cancer each year, the post-operative period represents a critical and historically undertreated vulnerability window. Standard of care after surgery has long relied on chemotherapy alone, with modest survival gains. A large Phase III randomized trial now offers a meaningful update to that picture, with implications for how oncologists approach the adjuvant — post-surgical — setting.

The CheckMate 77T trial enrolled patients with resected non-small cell lung cancer across a broad range of PD-L1 expression levels, randomizing them to receive adjuvant nivolumab — a PD-1 checkpoint inhibitor — versus observation alone following standard chemotherapy. The trial demonstrated statistically significant improvements in disease-free survival favoring nivolumab, with the benefit extending across PD-L1 expression subgroups, including patients with lower expression levels where immunotherapy has historically shown weaker efficacy. Overall survival data, while directionally favorable, remain immature at this analysis point.

This finding lands in a rapidly evolving adjuvant immunotherapy landscape. Atezolizumab and pembrolizumab have already carved out approvals in the post-surgical NSCLC space, but primarily in PD-L1-high or stage-specific populations. Nivolumab's demonstration of benefit across a wider PD-L1 expression range, if confirmed with longer follow-up, could meaningfully broaden eligibility criteria. The key limitations here are the maturity of overall survival data and the need to understand which subgroups derive the most durable benefit — disease-free survival, while clinically meaningful, is not the gold standard endpoint that overall survival represents. Additionally, the immunotherapy safety profile, including immune-related adverse events, must be weighed carefully in the post-surgical population.
