Endometriosis has long been dismissed as a localized gynecological condition, but its identity as a systemic inflammatory disease with measurable cardiovascular consequences is forcing a rethink of how diabetes is managed in this population. That rethink now has real mortality data behind it — and the numbers are striking enough to matter clinically.

In a propensity-matched retrospective cohort drawn from routinely collected health records, 6,144 adults with both endometriosis and type 2 diabetes were matched 1:1 and followed for a mean of 2.8 years. Those receiving SGLT2 inhibitors added to background metformin experienced all-cause mortality at a rate of 0.65% per year, compared with 1.15% per year in the metformin-only group — translating to a hazard ratio of 0.52 (95% CI: 0.38–0.72, p < 0.0001), or roughly a 48% relative reduction in mortality risk. Individual cardiovascular endpoints — including ischemic stroke, acute MI, atrial fibrillation, and heart failure — did not reach statistical significance between groups, suggesting the mortality benefit may be driven by mechanisms beyond discrete cardiac events, or that follow-up duration was insufficient to power those endpoints. Renal progression data were also captured, though the excerpt is truncated before those results.

SGLT2 inhibitors have already demonstrated mortality and cardiorenal benefits in large randomized trials like EMPA-REG OUTCOME and DAPA-HF, but those trials enrolled general diabetic or heart failure populations without specific attention to inflammatory comorbidities like endometriosis. This study fills a meaningful gap: endometriosis confers chronic low-grade systemic inflammation, immune dysregulation, and elevated thrombotic risk — a biological substrate that may amplify the anti-inflammatory and hemodynamic benefits of SGLT2 inhibition. The finding is clinically plausible and hypothesis-generating, but the retrospective observational design, even with propensity matching, cannot rule out residual confounding by disease severity or unmeasured lifestyle factors. The relatively short follow-up also limits conclusions about longer-term cardiovascular divergence. This is an incremental but genuinely useful signal that justifies prospective investigation in this underserved intersection of metabolic and inflammatory disease.
