Industrial fires represent a largely invisible gap in environmental health research, even as communities living near chemical and agricultural facilities face acute smoke exposures that differ chemically from typical wildfire events. A 2022 North Carolina fertilizer plant fire offers a rare natural experiment to quantify those risks — and the results suggest the health burden may be substantial and rapid in onset.
Analyzing hospital encounter data from four medical centers serving populations within a 25-mile radius, researchers modeled smoke dispersion using a Lagrangian trajectory atmospheric model and stratified ZIP codes into high, moderate, low, and no-exposure zones. During the fire week of January 31 through February 6, 2022, high smoke exposure was associated with a 78% greater rate of respiratory encounters across inpatient, outpatient, and emergency settings compared to unexposed populations (IRR = 1.78, 95% CI 1.25–2.57). Each additional day of any smoke exposure independently corresponded to a 28% increase in respiratory encounter rates (IRR = 1.28, 95% CI 1.11–1.49). Cardiovascular encounters showed a directional increase as well, though the respiratory signal was the more robust finding.
This work sits at an important intersection: while the wildfire-health literature has grown substantially over the past decade, industrial fire smoke carries a distinct toxicological profile. Fertilizer combustion can release ammonia, nitrogen oxides, and particulate matter with different size distributions and chemical compositions than forest biomass smoke, potentially triggering airway inflammation through different pathways. The study's reliance on a single fire event limits generalizability, and ecological modeling of smoke exposure by ZIP code cannot fully capture individual-level exposure variation. Nonetheless, the use of negative binomial regression to handle overdispersed count data adds methodological rigor. For public health planners, the findings suggest that hospital surge preparedness and early air quality alerts should extend explicitly to industrial fire incidents — not just wildfires.
