Amyotrophic lateral sclerosis remains one of the most devastating neurodegenerative diseases, with no cure and poorly understood environmental triggers. Whether industrial chemicals accumulated silently in human tissue over decades contribute to ALS onset is a question with profound public health implications — and this prospective multi-cohort analysis from Europe offers the most rigorous biomarker-based evidence to date on that question.

Drawing on pre-diagnostic plasma samples from 166 ALS cases and 334 matched controls within Denmark's EPIC Diet, Cancer and Health cohort — collected years before disease onset — researchers quantified circulating levels of 25 persistent organic pollutants (POPs), including polychlorinated biphenyls (PCBs), organochlorine pesticides, and polybrominated diphenyl ethers (PBDEs). Conditional logistic regression and generalized additive models produced a counterintuitive primary finding: several chlordane-related compounds showed inverse associations with ALS risk, suggesting higher pre-diagnosis plasma concentrations correlated with lower incidence. A meta-analysis incorporating three prior Finnish prospective cohorts partially reinforced these inverse trends. The notable exception was hexachlorobenzene (HCB), which yielded a positive association with ALS risk — a finding that stands out against the otherwise inverse pattern.

These results sit in tension with the broader retrospective literature, which has consistently linked occupational POP exposure to elevated ALS risk. The discrepancy is not trivial. One plausible explanation involves survivor bias and metabolic confounding: individuals who are biologically predisposed to accumulate chlorinated compounds may differ systematically from those who clear them faster, and that clearance capacity could itself be disease-relevant. The inverse signal may also reflect lipid redistribution or metabolic wasting in early pre-symptomatic ALS, altering apparent plasma concentrations before clinical diagnosis. With only 166 cases, statistical power is limited, and the non-significant trends for most compounds underscore the need for caution. The HCB finding warrants independent replication given its divergence from other POPs in the same analysis. Overall, this is an incremental but methodologically important contribution that complicates simple causal narratives around POPs and ALS.
