Medication errors and preventable adverse events remain a leading cause of death and disability globally — yet primary care, where the vast majority of patient encounters occur, has historically lacked robust evidence on which digital tools actually work. A rigorous synthesis drawing on 74 randomized controlled trials now offers the clearest picture yet of where technology delivers measurable safety gains and where it falls short.
The meta-analysis, published in BMC Medicine and covering RCTs from January 2001 through June 2025, pooled results across two principal outcome domains: medication safety process measures and non-medication process measures, plus adverse events. The medication safety arm, drawing on 54 trials, showed statistically significant benefits favoring digital interventions. The researchers compared single-component tools — such as standalone clinical decision support or electronic prescribing alerts — against multicomponent systems that layer several functions together, and separately assessed which specific design features most reliably drove improvement. Implementation factors including clinician workflow integration, alert fatigue, and system interoperability were explored through narrative synthesis.
This is not a typical incremental study. Seventy-four RCTs subjected to PRISMA-compliant, dual-reviewer extraction with random-effects modeling represents the most comprehensive evidence base assembled on this question. Its significance for health-conscious adults lies in what it implies about the infrastructure surrounding their care: digital safety nets embedded in a GP's software can meaningfully reduce the probability of a harmful prescribing error before it reaches a patient. The key limitation is that effect sizes and intervention types vary enormously across health systems, making direct translation to any single clinical context difficult. Alert fatigue — where clinicians begin overriding warnings reflexively — remains an underexplored confounder. Overall, this review is likely paradigm-reinforcing rather than paradigm-shifting, but its scale and methodological rigor position it as a benchmark reference for both clinical implementation and future research design.
