Psychological trauma following severe physical injury is a clinically underappreciated comorbidity — yet orthopedic surgeons rarely consider psychiatric pharmacotherapy within their scope of care. A randomized controlled trial now provides direct evidence that SSRI treatment initiated by orthopedic providers at the time of acute musculoskeletal trauma can meaningfully reduce PTSD symptom burden over the following year, challenging the conventional separation between physical and psychological trauma management.
Conducted at a single Level 1 academic trauma center between July 2021 and December 2023, the trial enrolled survivors of high-energy musculoskeletal injuries — explicitly excluding lower-energy events such as ground-level falls — and randomized them to nine months of fluoxetine (n=33) or calcium placebo (n=35). Groups were well-matched on age (roughly 40–44 years) and sex distribution. Outcomes were tracked at five time points across one year using validated instruments covering PTSD (PSS-SR for DSM-5), depression (BDI-II), anxiety (BAI), sleep quality (PSQI), pain interference (PEG), and social participation (PROMIS). Subjects in the fluoxetine arm demonstrated a statistically meaningful improvement in PTSD symptom trajectory over time relative to placebo.
This finding carries broader significance for two reasons. First, fluoxetine is already one of only two FDA-approved pharmacotherapies for PTSD (alongside sertraline), so the mechanistic rationale is established — what's novel here is the delivery context: orthopedic, not psychiatric, providers initiating treatment. Second, the window of intervention — the immediate post-traumatic period — may represent a critical neurobiological opportunity to attenuate fear-memory consolidation before PTSD becomes entrenched. That said, the trial is notably small (68 participants total), non-blinded, and restricted to one institution, which limits generalizability and introduces potential bias. Effect sizes and dropout rates from the excerpted data remain unclear. Still, for a largely ignored intersection of trauma care, this represents a clinically meaningful signal worth replicating at scale.
