Across 126 studies enrolling more than 8 million participants, greater adherence to the Mediterranean diet was associated with statistically significant reductions in risk for seven site-specific cancers. Effect sizes ranged from a 17% reduction for oral cavity cancer (RR = 0.83) to more modest but consistent reductions for colorectal (RR = 0.95), breast (RR = 0.95), stomach (RR = 0.93), liver/gallbladder (RR = 0.94), bladder (RR = 0.96), and head and neck cancers (RR = 0.88). Cancer-related mortality also fell with higher adherence (RR = 0.97). Evidence certainty was rated moderate via the NUTRIGRADE framework.

The numbers themselves are modest — a 3–17% relative risk reduction per adherence point — but the breadth of cancer sites implicated and the sheer cohort size make this one of the most comprehensive dietary oncology meta-analyses to date. The mechanistic logic is credible: olive oil polyphenols, dietary fiber driving favorable gut microbiome shifts, and anti-inflammatory omega-3s from fish collectively suppress pathways — NF-κB, IGF-1 signaling, oxidative stress — implicated across multiple tumor types. Crucially, this remains observational evidence; Mediterranean diet adherents tend to have healthier lifestyles overall, and residual confounding is near-impossible to eliminate. No randomized trial of sufficient duration exists to establish causality definitively. Still, for public health guidance, moderate-certainty evidence across 8 million people supporting a broadly accessible, affordable dietary pattern carries real policy weight. This finding is confirmatory but meaningfully scaled — and practically, the case for recommending Mediterranean eating to reduce population-level cancer burden has rarely been stronger.
