Vision loss from age-related macular degeneration has long been framed primarily as an eye disease, but a large longitudinal analysis now positions it firmly within the broader landscape of systemic metabolic and vascular dysfunction — a framing that could reshape how clinicians screen and stratify risk in middle-aged and older adults.
Drawing on nearly 320,000 UK Biobank participants followed for a median of 12.5 years, researchers tracked incident AMD across four stages of cardiovascular-kidney-metabolic (CKM) syndrome — a relatively new composite framework recognizing chronic kidney disease, cardiovascular disease, and metabolic risk factors as an interconnected disease continuum. Compared to individuals at CKM stage 0, those at stage 1 showed a 15% elevated AMD hazard (HR 1.15), rising to 25% at stage 2, 22% at stage 3, and 48% at stage 4 (HR 1.48). Nearly 5,000 AMD cases accumulated over follow-up, providing substantial statistical power. Notably, when CKM stage 4 was combined with high genetic predisposition to AMD, the joint hazard more than doubled relative to those with neither risk factor, though the interaction between genetic risk and CKM syndrome did not reach formal statistical significance.
This study contributes to a growing body of evidence linking retinal microvasculature health to systemic cardiometabolic status — the retina, after all, shares hemodynamic and metabolic vulnerabilities with the kidney and heart. The graded dose-response relationship across CKM stages strengthens the case for biological plausibility rather than confounding. However, several caveats deserve emphasis: this is a retrospective, observational cohort with AMD ascertainment via medical records rather than standardized imaging, which likely undercounts early-stage disease. UK Biobank participants also skew toward healthier, predominantly white Europeans, limiting generalizability. Critically, CKM syndrome is itself a composite endpoint, making it difficult to isolate which metabolic or vascular components most directly drive retinal pathology. Still, the findings add meaningful support to the idea that managing cardiometabolic risk factors could carry ancillary benefits for preserving long-term vision — an underappreciated dimension of systemic disease burden.
