Hereditary breast cancer risk is not a single genetic story — the mutations that matter depend heavily on the tumor's molecular subtype, and conflating HER2-positive with HER2-negative disease in genetic counseling may leave meaningful risk signals undetected. A decade of multigene panel testing data from nearly 1,700 Chinese patients now sharpens that distinction considerably.
In a retrospective analysis spanning 2014–2024 at Sun Yat-sen University Cancer Center, germline pathogenic variants (GPVs) were detected in 18.1% of HER2-positive breast cancer patients compared with 22.4% in HER2-negative cases — a modest but clinically relevant gap. More striking were the compositional differences: HER2-positive carriers showed a dramatically elevated TP53 prevalence (26.9% vs. 3.3% in HER2-negative carriers) and a BRCA2 predominance (44.2%), while BRCA1 mutations — the canonical hereditary breast cancer variant — were far less common (11.5% vs. 43.5%). HER2-positive carriers were also diagnosed at younger median ages and more frequently presented with hormone receptor co-positivity, despite less frequent family history of BRCA-related cancers. Notably, GPV status did not independently predict locoregional recurrence or distant metastasis in HER2-positive patients.
This dataset reframes how hereditary risk panels should be interpreted for HER2-positive patients. The high TP53 burden in this subgroup is biologically coherent — Li-Fraumeni syndrome pathogenic TP53 variants are known to associate with HER2-amplified tumors — and suggests that standard BRCA-focused counseling frameworks may underserve this population. However, the retrospective, single-institution design and the East Asian patient composition limit direct generalizability to Western cohorts, where germline variant frequencies differ. The finding that GPVs don't worsen HER2-positive prognosis is reassuring but requires prospective validation, particularly as HER2-targeted therapies like trastuzumab and pertuzumab may differentially interact with DNA-repair-deficient backgrounds. This study reads as confirmatory of emerging subtype-specific genetic patterns, with meaningful implications for refining multigene panel counseling protocols.
